If your complaint is about the assessment, the prescriber's decision or your clinical care, it is handled by the clinic. If it is about dispensing, the medicine supplied or delivery, it is handled by the pharmacy. If you are not sure, contact either and they will route it.
If you are not satisfied with the response, you can escalate to the relevant regulator: the Care Quality Commission for the clinic, or the General Pharmaceutical Council for the pharmacy.
The clinic and the pharmacy each keep their own records of your care and are each responsible for protecting them. You can ask for a copy of your information, ask for it to be corrected, or complain about how it is handled.
